Personalized Banner Requests

So you want a cool banner to show off your Guild Character? No problem! I'd be happy as hell to make you one!

All's, I need you to do is something very simple for me. Below is the the form I'd like you fill our for me, and go ahead and post in this thread. I will have 5 spots open at a time, and once they are completed; there will be a complete post, with links to all of the banners. :3 Simple right?

Here is where you will find your & others completed banners. This is also where you will want to check if you want to see an example of the works. Look for your User-Name & You'll find your banner! Upon completion, I will also edit your posts and strike through your requests -- this will help myself be more organized. Banners will stay on the "Available For Pick-Up" List for Approx. 2 Days, and then will be moved to "The Final's List". (So if you miss it, look for it there.)
