Well it is nice to meet the three of you. I guess the best way to start this off is to learn the way our guilds function. How does your ranking system work? We run pretty close to Naruto, the only differences are the addition of a medical nin ranking system, as well as removing the academy level.

We don't exclusively do ninja combat. In fact, most of our users are from a wide variety of backgrounds.
Essentially, I am the 'Kage' equivalent to your system.
My "Three Kings" act as my right-hand, and delegate in my absence.
Beneath them, we have the Dukes, who are members chosen for their dedication and skills to act as peacekeepers, and extra eyes for myself and the Kings. They also act on our behalf if assistance is needed with something.
We do have a training area, for members to practice and spar. We also hold tournaments. We don't have a medical division; our philosophy is one of independent loyalty. You fight for the clan, but are strong enough to stand on your own two feet.

So you clan is kind of a quasi-European system.
How do you choose your Dukes?
It is an interesting philosophy. Our clan helps train our members to work in four man cell teams. We do have many times when we have to fight alone. Chaos, Masa, and I can attest to that fact.

My Dukes are chosen based on their activity and character. I see how many members I have to determine how many Dukes I feel we need, and then I review candidates based on their merit and distinguishing traits. Those I like, I make Dukes.
I've done the three and four man groups in the past, but I find it to be a system that is quite difficult to maintain. Teaching partnership requires all members to be on for the best results to be achieved. In many cases, that can't always happen, so the whole group suffers for it. Instead, I prefer each member train individually, and be capable of handling multiple attackers at once. Better to have one person that can fight off four people than four people who can fight as one unit. Makes them into sharper fighters.
