If you have the walkthrough you can take the Ragnarok or Balamb garden and travel around. Depending on how your abilities are, level of GF, level of magic and your weapon upgrades I can tell you the best places.
If you have the Ragnarok you can go to every place. If not you have to refer to the bottom portion of this.
1. Deep Sea Research Center
It is not quick unless you know what you are doing. Make sure you are fully equip with magic, especially Curaga, Regan, Aura and Protects. You will encounter red dragons which are a pain to fight but they will give you a good amount of EXP along with AP. You will find them often there and make sure you also stock up on FULL LIFE.
2. Trabia Garden
There are Snow Lions that are an easy 5-10 minute fights that will give you sufficient EXP and a decent amount of AP. Junction Fire in your attack and raise compatibility with your character that has Ifrit junctioned. You will use him at least three times in one fight. Also make sure you junction Blizzaga as your defense.
3. Esthar
Elnoyl are also good monsters to fight. They always have a pattern. They are similar to the Elnoyl in the beginning of the game in disk 1, however I believe they have different statuses.

4. Remember the training area. If you find T-Rexar, cast blind on him and then gave fun with summoning Shiva at least 3-5 times with boost. Easy EXP and AP. Grats though will be the most common but they are also quick 1-5 hit kills.
Fights to avoid!!!!
DO NOT FIGHT TONBERRYS UNLESS YOU ARE QUESTING FOR TONBERRY KINGS, they are a waste of time with gaining EXP and AP because they only offer AP and NO EXP. Stay Away!
