Humm... hard to say. Each MMO I've played had some kind of upside...
I'd say MU-Online.
I got on for 5 minutes, there were no tutorials in sight, the website and forums were no help, and I had no idea what I was doing while slaying giant bulls or something.

I would say for game play...I had a bad experience with Dofus, for whatever reason I had a hard time maneuvering in that game.
Player wise... Maplestory, when I started playing there were lots of very unfriendly users which I think was part of the reason I decided not to play it very long.
Bots/Lag...Silkroadonline regretfully, it's a really nice game and I am a big fan of it, but they have strayed off the pathway and let the bots control the game.
Translation...Neosteam, please if you can just download it and get to like level 10 or something and then you can quit, but the translation is horribly amusing, since you would have to be really bad at English to mess/mix up some things (like buying and selling).

Never played Neosteam (love to though), but it is still beta, right? It should be expected to have translation errors.

I'm not dissing the game... I think World of Warcraft, because, I don't see it being worth $15/month to play.
There are more than enough Free to Play MMO's that offer all WoW does and some offer more

For worst MMO i would have to give the SAGY (If you use screwattack you will know what i mean) To Manga Fighter
Manga fighter lacks
Character custermisation(Terrible spelling)
Community
Smoothness (To much Lag)
Fun for a long period of time
Graphicly terrible

The Unknown Mercenary runescape cause of crapy ness....or....world of warcraft because of crap price i agree they both suck

incase you didn't know.. runescape is a browser based game AND it's coded in java script & doesn't even run on a game engine, so you gotta cut it some slack(YOU try making a mmo which supports hundreds of thousands of people at any one point in time on java script), ... & on the upside, it has gotten an uber(compared to the old graphics) graphical update although the loading time's are a bit slow. maplestory is crap on account of the dev's being lazy & making a CLIENT based game that looks like a 2D flash game. At bare minimum, they could've atleast used some source code from an old korean mmo or something. only reason people even play it is cuz it's free. WoW is a rather nice game but what drove me away was the subscription price as some, if not, most would agree.
As for worst MMO IMO? Maplestory,OR a few notable text-based mmo's which, even then, i like better than maplestory.
