I am thinking of coming out to my parents on my birthday, November 16th. For those that just stumbled across this journal, I am male-to-female transsexual. I am 17 years old right now and hoping to get some ideas on what to do to come out to my parents. I just need ideas that will hopefully make my parents accept me for who I am.

The only advice I can give is to be sure to take the time to push your emotions into the background for a bit and really analyse the situation you are in with a clear mind. How well do you know your family's feelings about such things? Has the subject ever come up in general conversation? Have they expressed any opinions about people who are transgendered? Was it positive, or negative? It's very rarely neutral Are there strong religious or conservative trends in your family? Are the fairly liberal?
Also, be careful of your timing. If you just drop this on them they may react more negatively than if you gentle them in to it. Not knowing your specifics (the livejournal link just takes me to a blank page...) I can't give specific suggestions, so all I can recommend is to try to feel out the general attitude your family has and be easy in your apporach. BTW, are there differences of opinion between members? Can you maybe get some of your family on your side before you tell the others? That might help too, moral support and all that.
Anyway, if things seem less tha ideal, don't be afraid to consider waiting until you are more secure on your own two feet. Being kicked out at 17 is worse than being silent for a bit longer.
