I had this sitting in my game folder, and wanted to get everyone to fill one out, and this seems the most expedient way to do so. Answer as your character would. This is intended for a Serenity style game, but it's generic enough to cover other games. (question 22 may be the most specific)
You don't have to type in Malspeak if you don't want, but it may inspire you.
Also, even if you're not in my game, feel free to make up a character you'd play in the 'Verse. I may even use the characters in game.

To get the ball rolling, I'll start with Captain Jonathan Burke. I may do Jurgen later.
1. What is your name? (Why is this your name? How’d you get it? Any nicknames?)
My name is Jonathan Burke. It's my name, well, cuz that's what mom and dad decided to name me. As for nicknames, well, no. I don't go by nicknames. My name is Jonathan, regardless of what Kera says.
2. What do you look like? (How old are you? How old do you appear to be?)
I got brown hair, blue eyes, stand about 6 foot, give or take, and I'd like to think I'm in pretty decent shape. I'm 29 years old, but when I look in the mirror, well, I figure I look older. Life plying the black'll do that to ya.
3. What is your style and attitude? (How do you dress and talk? Are you cultured or backbirth?)
Can't really say I've really got much of a style. T-shirts and jeans work for me. Always gotta have a good pair of work boots. Nothing gets you dead faster than screwed up feet. I talk like most other folk out here, but I can be cultured if I need to. I'm just full of surprises.
4. Do you have any quirks we should know about?
Why would you want to know? Does it really matter? Guess when you get down to it, does a sweet tooth count as a quirk?
5. What kind of impression do you think you first make?
Don't rightly care one way or t'other. Folk're gonna think what folk're gonna think. They don't like me, that's their own problem.
6. Where do you hail from? (Where were you born? Where did you grow up? What was your childhood and youth like? What does your family think about you?)
Well, I was born on Londinum, and spent most of my early childhood on the Persephone skyplex livin' with grampa S. Don't really remember much of my childhood, but I can't rightly say it was bad, cuz it wasn't. As fer what my family thinks of me? Mom and Dad ain't 'round no more to ask, and, well, Kera ain't an impartial party, Dohn Ma?
7. What is your best quality as a person? (What is your worst quality as a person?)
My best quality? Like to be my stunning personality I'd say. Jokin' aside, I guess it'd be my ability to lead. F'r some reason, folk listen to me when fists and firearms start firin'. Worst quality? I try not to think much on it myself, but I gotta admit, I got a tendency to fly off the handle at times. Not always good f'r diplomacy if'n you follow.
8. Describe your personality in only two words.
Determined Leader. If you ask Kera, likely she's got a few other choice phrases in mind.
9. Describe your favorite things and your least favorite things? (food, drinks, books, music, assault rifles, etc)
Favorite things? First off, real food. Don't much care what kind, so long as it ain't protein paste. I got a weakness for good whiskey and bad movies, and the old Earth-that-was Punk music really does something for me.
10. They say that you can judge a man by the enemies he makes and the friends he keeps. Who is your “nemesis” and who are your allies?
I'd prefer not to get into it, but I got a few too many of the former, and not enough of the latter. What allies I do have, I'm real thankful for. They've kept me out of the ground more times than I can really count. I owe them big.
11. What is your romantic life like? (Orientation? A virgin? Has there been a “great love”?)
My romantic life? Ain't got much of one at the moment. Last one went and got herself killed in some lab, and the one before her got nuked on Ita. Way I see it? I think I'm cursed. As for the great love thing? I'd prefer not to get into it.
12. What do you fear? (Do you have recurring nightmares or dreams?)
I don't rightly like being alone personally. Now, that ain't to say I gotta go and attach myself to someone, but, well, you spend three weeks stuck in a disabled fighter, the cockpit of which ain't much bigger than a coffin, watchin' the lifeless bodies of your closest comrades float and bang into your windshield, and see how much you like bein' by yerself after that. I'm done talkin' on this subject, can we move on?

The Black Fox. Don't know who he is? Go look 'im up. You can work a Cortex terminal, right?
14. What offer would be irresistible enough for you to betray your friends or crewmates?
I ain't 'bout to talk on this one. My crew is my family. I won't. I let my family down before, and I ain't gonna do it again.
15. What principle matters to you the most?
Once I figure that one out for my own self, I'll be sure to speak on it with you sometime.
16. What is your most treasured possession? Why?
It's gotta be my flight goggles. Don't ask why, it ain't your business to know.
17. Who do you value most in all the ‘Verse?
Crystelle. She's the most important thing in my life. No one, but no one, gets between me an' her.
18. What are your ambitions? (What are your social, professional and romantic goals? What do you wish you could do, but can’t?)
I ain't really got much of a goal beyond stayin' free, stayin' flyin', and keepin' out of the Feds hands. Askin' fer more'n that is just beggin' for trouble, and trouble find me and mine on a pretty regular occurance already. No need to egg it on.
19. Where do you picture yourself 20 years from now?
Honestly, the way I'm goin', like as not, I'll be in the ground. I'd prefer to stay breathin' and walkin' myself. Crystelle ain't gonna be walkin' herself down the aisle, right?
20. What other kinds of work have you done to pay the bills?
All kinds my friend, all kinds. I do what needs to be done to keep myself flyin' and my belly full. I stop short of murder-for-hire, there's so many folks out there better at it than me. I happen to know a couple. Way I figure, the fact I pay him keeps me off his list. Leastways, that's what I'm hopin'.
21. What are your political and religious beliefs, if you have any? (Do you have any prejudices?)
I think the Alliance needs to take their overinflated sense of "enlightenment" and cram it right up their backsides. But, that ain't like to happen, cuz as we all know, stupid is forever. I don't really have much to say one way or t'other bout those of religious bent. Myself, I ain't much for faith. If I had any to begin with, I lost it in the Battle of Sturges.
22. What were you doing during the Unification War? (How has the war affected you? Did you serve in the war? Which side? Did you lose anyone you cared about in the war?)
I was fightin' for the side that had it right. No one, not even the Alliance has got any right to keep folk from bein' folk. I lost a lot of good friends, I even lost the respect of my little Mei Mei for runnin' out on her. I don't rightly blame her m'self. I just wish I coulda had more influence on things. Might'a gone diff'rent, you know?
23. Do you have any hobbies? (How do you pass the time away?)
Needlepoint. Nah, seriously, I kinda dabble in a lotta stuff. I like a good game of Tall Card now an' then, usually when I wanna unload some chores on Red. Got a good collection of old Earth-that-was "B Movies" too. Can't get enough of those horrible titles and bad acting.
24. What event in your life that you did are you most proud of? Most shameful of?
I'd like to say the birth of my daughter, but, I wasn't there for it, and I wasn't even aware she'd been born til she was six. What's the odds I'd find her, sick and dying, on a ruined little planetoid at the edge of the 'Verse? Some higher power must've liked me, or her. Or maybe they hate her, considerin' my luck, I'd hate for her to lose another parent. I gotta stop talkin' on this subject, I'm gettin' all teary. What? Most shameful? I don't wanna talk on that one either. Next question.
25. What have you been doing the last few months?
Flyin' the black and plyin' my trade. Can't really ask for more'n that.

This would be the answers for Velvet, my Serenity character.
1. What is your name? (Why is this your name? How’d you get it? Any nicknames?)
My name is Velvet Cho. It is the name my mother gave me. I have no nicknames, but it would not be advised to call me Vel.
2. What do you look like? (How old are you? How old do you appear to be?)
I have long straight black hair that when in daylight shines blue. Thank my Asian mother for that. I have a very nice figure, which I keep up with doing various activities. My strongest feature would be the color of my eyes. They are the color of amber - or good strong Cognac.
My age? A lady never tells.
3. What is your style and attitude? (How do you dress and talk? Are you cultured or backbirth?)
As a companion, I dress to what suits my client's fancy. Therefore, my closet is quite large. Normally, I prefer flowing, soft materials. Comfort is something of a must for me. As for being cultured, that comes with experience and the right connections. I was born a farmer's daughter after all.
4. Do you have any quirks we should know about?
I am meticulously clean, and I try to keep my surroundings that way as well. I also have a passion for food. I can be a wonder in the kitchen. I am, to my own slight disgrace, a tri-plex lover. I have not met one yet I have not conquered. And lastly, I start to feel quite uncomfortable if I have to remain in one place for too long. I do enjoy my space. Anything else, you'll have to find out for yourself.
5. What kind of impression do you think you first make?
I'd like to think a memorable one. It is part of my job after all. A companion who can not take center stage when needed is not worthy of the title. I can come off as a bit stuffy, and quite possibly arrogant, but I do not hide these flaws from myself.
6. Where do you hail from? (Where were you born? Where did you grow up? What was your childhood and youth like? What does your family think about you?)
I was born on Lilac. Reavers were not an uncommon threat, and thus my family did eventually fall prey to them. Living on Lilac was no picnic. If my parents were still alive I do not think they would be happy with my line of work. However, it is a little late for remorse on that point. Nor do I feel any.
7. What is your best quality as a person? (What is your worst quality as a person?)
Best quality you ask? Well of course my physical appearance. Again, I can only stress the significance of my line of work. If we delve deeper then that, I'm afraid I can not answer. My worst quality, well that is something else that remains to be seen.

Valuable asset.
9. Describe your favorite things and your least favorite things? (food, drinks, books, music, assault rifles, etc)
My favorite book would have to be the old fairy tale of Peter Pan. The childlike thought of never having to grow up is quite appealing. I prefer soft, mellow music. It helps to clear my mind, and occasionally gets me in 'the mood' so to speak. Food is easy, and I might say a little out of character for one such as myself. Give this girl a greasy cheeseburger and she's a happy companion. As for weapons of choice, no companion should be unprepared if something goes wrong. I keep my needles ready, just to make sure a client who gets a bit rough falls asleep peacefully, unharmed. I also carry a small pistol at my thigh if things really go south.
10. They say that you can judge a man by the enemies he makes and the friends he keeps. Who is your “nemesis” and who are your allies?
I'd like to think of my allies as the people I am currently traveling with. They do seem to be a rowdy bunch, but they seem like decent people. I also have a few higher ups who will do me a favor every now and again. As for my enemies, they know who they are. That's all I'm willing to divulge.
11. What is your romantic life like? (Orientation? A virgin? Has there been a “great love”?)
Romance is part of my way of life. I am a certified companion, so being a virgin is a quite laughable statement. There has been no one true love, for I do not believe such a thing exists.

My fears are my business.
13. Who do you look up to? (If there isn’t anyone now, was there someone in the past?)
Shaydra, my mentor. She taught me all I need to know and I respect her opinion and advice as I would a mother. There was one other person as well, but that's none of your business.
14. What offer would be irresistible enough for you to betray your friends or crewmates?
Everyone has a weakness. If someone were to find it and exploit it, ones survival kicks in. However, nothing of monetary value would make me betray the people I consider family or friends. Nothing material at all.
15. What principle matters to you the most?
Respect. If one loses the respect of his/her crew or comrades then it's no longer worth the fight.
16. What is your most treasured possession? Why?
A picture. That is all.
17. Who do you value most in all the ‘Verse?
Freedom. There is no who.
18. What are your ambitions? (What are your social, professional and romantic goals? What do you wish you could do, but can’t?)
I would eventually like to be financially secure. I would like to travel at my own pace, not having to worry about my next job constantly. When it comes to love, I feel I'm a bit jaded. I'm not sure if those feelings ever really existed in me.
19. Where do you picture yourself 20 years from now?
In twenty years, I'd like to think happy. As for anything else, one should not speculate.
20. What other kinds of work have you done to pay the bills?
Things I'm not willing to discuss. There was a period of time where selling my body was not as glamorous as it is now.

I am all for peace, but that is a naive notion. I am highly opinionated, and when you get on my bad side, it will not be pleasant. As for god, to be frank, ******** him.

I feel this question bears no significance at this time.

I pass the time by cleaning and cooking. I am a companion with a housewife's skills, but I lack the discipline.

I am most proud of the day I became an official, licensed companion. I'd rather not talk about anything else.

Doing what I do best. Making money.

My name's Corneil Redfern *waits for the laughter to die town* Friends call me Red because of my hair and it's part of my last name.
2. What do you look like? (How old are you? How old do you appear to be?)
I got long, blood red hair, one green eye and one blue eye which sometime freak people out. I DO shower, but I just don't wash my hair often unless I'm on a date or something. It takes too damn long to wash. I'm about 6 foot tall and pretty lanky, but a pilot doesn't need to be a walking muscle.

Brown coat, brown coat, and let me think...brown coat. I don't leave the ship without it. Usually in jeans and a tee shirt too. As far as cultured or backbirth...yeah me trying to be very cultured is like trying to pull a Crazy Ivan in a mantis class ship, just doesn't work.

I'm from Persephone, spent most of my childhood as the "weird kid with the creepy eyes." Enrolled in the Indepentent army as soon as I possible could. My parents? Not much to talk about there, Mom's an ex-pilot who only did transports and Dad sells "hot-dogs" around Persephone. Mom's really proud of me, but Dad kinda snapped recently and now things to walls talk to him in some language called Spanish.

Best quality? Hmm...I guess I'm a good friend, ask around the crew and see if anyone hates me. Worst quality? Well they say that I have the intelligence of a rever with a bullet in the brainpan...

My favorite thing is flying. Nothing feels better then bringing a ship through the verse. Well, almost nothing...that's a story for either another time or later on in this thing. Least favorite thing would be cat...I hate cats...

My "nemesis" was turned into sauce, so none that I know of. My Allies are my crew, especially Amelia

Love life? I'm trying to get one going...but with all the crap that's been going on it's hard to even get a minute alone with her. When I did last time Kera and Haywire followed us and ruined it for both Amelia and I. I need to figure out a way to let her know I really care for her...

The mechanic, the guys got a messed up brainpan...also afraid of loseing a few certin crew members.

My mom, she's an awesome pilot even though she's retired now.

I don't really have many ambitions, other then to keep flying for the rest of my life and I hope Amelia will someday be with me.

Either dead or settled down.

Random odd jobs, like delivering packages on a bike and crap like that.

Not big on religion, but not against it either. The Alliance takes it a bit far though, I heard they have people who believe in them so much they'll do anything.

The Captian and I were fighting on the same side, I was a fighter pilot. I would rather not talk about that though.

Hanging out with Amelia, playing with action figures...not dolls, action figures. I like gambling, but that sometimes gets in into problems. I also learned to never play Tall Card with the captian, he's damn good at it.
24. What event in your life that you did are you most proud of? Most shameful of?
Most proud of? Out piloting 10 to 25 year vets of the war at the age of 18. You should have seen the look on those old farts faces. As for most shameful, though the end result gave a crew member a cool new hand, I blew up a complex and nearly killed half the crew...they give me the stink eye now when they see a granade on my belt.

Trying to work up enough courage to tell Amelia that I love her.

My name is Dr. Steven Tardis. Never really asked my parents about my name. Nicknames...well lets just say if I catch anyone calling me Dr. Tar I'll slap them in the goram face.

I am about 5'6'' with short brown hair and green eyes. Im 28 years old.

Most of the time I wear my doctors coat with jeans. How I talk...you will learn that soon enuff you chween. Dont let my mouth fool you, I come from Osiris and am quite cultured THANK YOU.

Well, my hair is shorter now. Itls black enough that it shines blue in the light. My eyes are blue too. 5'4 and 120lbs soaking wet. I look alot weaker than I am.

I can be kind when need be.
I wear japanese tops, tight leather pants that pull into combat boots. Nothing special.

Belrephon(Sp?) Home to the rich and paranoid. I grew up having anything I ever wanted and I have to say that it's not that great. I miss my friends though.
My mum is always too drunk to care and my pa is ashamed of me right now. But he needs me for financial reasons.

1. What is your name? (Why is this your name? How’d you get it? Any nicknames?) Hey 'dere, name's Harry Callahan - Haywire to da crew 'dere! Got that name from 'ma schoolin back on 'ol Saint Albans. I was a bit of a techie as a munchkin and I fiddled takin' stuff apart a really shouldn't 'ave. A few nasty zaps and I got me a fancy nickname!2. What do you look like? (How old are you? Ok, bein' a radio junkie, I got my hands all kinds a' Earth-Dat-Was tunes and man I gotta tell ya- It's gotta be loud, it's gotta be fast and it's gotta make ya feel like da inside of yer head's gonna explode! Dat's my main fix but ya know there's so much out 'dere dat I'm too particular in da long run. If it's got a beat, I'll give a ear and I like just 'bout most of it! Hmmm weapons, well anthin' that keeps them and their bullets far away! Thanks ta that lousy Alliance Conscription Training I got a mean shot with a rifle. Had ta use 'em ta hunt game back home too so it only works. I got a decent trigger with a shotgun ta keep the uglies who get too close from gettin' closer too.
But ya know when it all comes down too it, as long as I got my wires and knobs and enough folk with da patience ta call me friend and listen den 'ol Haywire's doing fine! I can't 'magine he's too happy 'bout me givin that order ta retreat from Serenity Valley. Frankly I 'aint either and I learned it ain't the name of the force ya fight in, it's the folk in it. Some Brown can be just as hypocrite as the purple-bellies and I ain't gonna track with them if I can help it.
Only other guy I can think of is the lanlord from back on Saint Albans. Ya got 20 some odd Callahans livin' in a space no bigger than a shack and 'ol pappy finds his own mine claim. Purple belly landlord says claim's his 'cause pappy used his mining gear. Ha, 'ol "Havoc" Callahan gave is soul on that claim just ta get the few credits he could ta keep his family happy. I tell ya once I get the coin, I'm cartin' the clan off of that rock! So long as that mook don't pull nothin' funny...11. What is your romantic life like? (Orientation? A virgin? Well back home ain't too many ladies ta court to a poor miner's son and well I had a few romances in ma younger years. Trekin' 'round makes it kinda tough ta find the soul mate in the 'verse and crew relationships can get ugly if ya gotta take or give orders ta the one who keeps ya warm at night.12. What do you fear? (Do you have recurring nightmares or dreams?) Dead in the water... 'Aint nothin' worse than bein' on the boat when it all goes dead and yer sittin in the hand of the creator! Out in the Black, ya ain't got nothin but yer withs and gear and when one fails, the other ain't too far behind! I got me a pricey set a' beacons I keep on deck for just such an occaision! Only other stuff that gets under Haywire's skin is surprises. Ya know, the kind that can leave ya six feet down? Traitors, ambushes, explosions ya don't directly cause! I keep my ears out at all times for such events and I'll be damned ta death itself if I let anybody get da drop on 'ol Haywire! 13. Who do you look up to? (If there isn’t anyone now, was there someone in the past?) Ha! When it comes down to it, ya gotta admire yer crew! They're the only folk who can deal with ya on a daily basis fer months at a time and not put a knife in yer back!
Anybody with the cans ta lead a group of nuts through tha 'Verse, hell the capn' of the ship is always somebody ta respect! He's crazier than you fer one and he's always got alot on his plate tryin' ta keep the food and credits fresh on it. Johnothan's a good guy dere and I do what I can ta make sure I ain't part of tha problem!
Amelia's a good kid who's got more on her shoulders than anybody'd give her credit for! She's got a big burden on her small shoulders keepin the crew from lookin' bad bein' public relations and all. It'd be a big surprise to anybody who didn't know that she could lift a moose with those shoulders there!
Red, well that kid's seems like he's been round tha block for a youngn' like him. The way he sters that boat, you'd think he knows it by name and tucks it in at night! Poor guy though. Fer all his flyin skills and reaction time and what not, he can't see what's right in front of him some times. Amelia's got her hands full if the two of them don't get past the whole problem of being young.
The mechanic... not much fer him bein' how he keeps to himself but I gotta hand it to him, he keeps that boat flying on love and grease... lots of grease. He knows his stuff and he might a bit rough 'round the edges but ya gotta take what he's been through ta 'count.
Doc Tardus, he's another rough cookie with a peronality as comforting as his bedside manor. But the ol doc has a few tricks up his sleeve that make ya never want to count him out of any scrap!
The new companion, Velvet, why she hasn't tucked tail and run, I'll never know. But hey, since her cookin' keeps us all alive and happy I'm glad she ain't gone yet!
That leaves Varkel and Kira... Don't stand in front a Varkel, gun or not and don't stand in front of Kira, big fancy gun or not... Never really thought too much on that. Hopin' ta just live long enough ta see old age and still have a boat ta be cartin' around in 'till the ol' ticker stops I guess!19. Fightin' Alliance as a Brown Coat, right smack in Serenity Valley. Lost allot a close friends and loyal crew, had ta relay a few orders I wasn't proud of, had to take a few hits. All in all it hurt the heart pretty hard but ya gotta fight fer what ya belive in. Specially if someone's tryin' ta take it from ya!23. Do you have any hobbies? (How do you pass the time away?) Mostly listnin ta radio for anythin' and everythin' out there in tha Black. I like tinkerin with things with flashing lights and pretty buttons, specially knowin how they work! Leavin' home and tellin' the Alliance where ta stick it, probably the bravest and dumbest thing I ever did! They weren't gonna keep me down!
