Harry Potter:

Profile Explanation & Helpful Hints:
The most important thing to remember, throughout this whole profile, is to always explain why. Example: don't just list names for who their friends are, give a reason why they are friends.

AU or Canon: Does the story stick pretty close to the books or is something altered? Like does Harry end up with Luna instead of Ginny? Having an OC does not automatically make a story AU.
Name: First, middle, and last is great but really just a first name is fine. Also nicknames and aliases should go here.
Birth date/Era: You don't need to know their exact birthday but a general idea is helpful. Era should be obvious although it does not have to be specific either.
Work/Schooling: What school did they go to? If it was Hogwarts, then what house were they in? Were they a prefect? Head boy/girl? What job do they have if they have one? How do they feel about work/school?
Family: Describe their immediate family: mother, father, siblings, and even grandparents if needed. You don't need much detail on them but sometimes having the information in the background can help.
Blood status/Lineage: Blood status should be obvious and lineage is to address the extended family. Are they related to a long line of such and such people? Does their family name go back generations? Does their lineage have any effect on them? How do they feel about their heritage?
Home: Where do they live? Do they live alone or with others? Describe the basic aspects of the house and grounds if you like, say whether it's messy or clean, etc. A person's home can say a lot about them.
Appearance: List things like: hair color and length, eye color, height, weight (you do not have to be exact), physical build, body type, prominent feature, nationality, skin tone, distinguishing marks, etc. How old do they look as opposed to how old they are? What's their fashion sense? Do they look like anyone (related or not)? Are they healthy? Are they healthy looking? Do they take care of themselves? What do they think of themselves? What's their favorite and least favorite things about their appearance?
Personality: This is the place to list how they act, what kind of person they are on an average day and throughout the story. You can include things like how they relate to others, what puts them at ease, what aggravates them, what are their priorities, how do they feel about themselves, what are their soft spots, what won't they tolerate, what are their habits, how do they treat others, how do they treat themselves, how do they think others perceive them, what are things they want to or don't want to change about themselves, how would they act in a crisis or on a normal day, etc.
Special Skills: A special skill is something that someone is really good at, better at than most other skills. If the character gets very high marks in a certain class that would be a special skill, just the same as if they were very good at a certain activity or sport.
Weaknesses: These are the opposite of special skills and consist of things they are particularly bad at. Not being able to swim would be a weakness just like if the character had a trait like a short temper or was so overly curious it got them into trouble. It is important to remember that one does not need to "balance out" their characters special skills and anti skills. Just list what you think is realistic for your character and we'll go from there.
Romantic Interests: This is only important if there is some aspect of romance in the story. Remember to list why the person(s) are of romantic interest to the character.
Enemies: All characters have enemies, even if only minor ones. Remember to explain why someone is an enemy to the character, perhaps add how it all started and if anything could change their feelings for each other.
Friends: While not everyone has friends, this can also be a place to list acquaintances and the like. Remember to explain each relationship.
Hobbies: What are the things they do in their free time? Remember to explain why they do and/or enjoy these things.
Allegiance/Motivation: What side are they on and why? What is the driving force behind their decisions?
Hopes/Fears: What do they wish would or would not happen? What are their secret wishes? Do they have an particular phobias?
Goals: List their short term and long term plans. How do they plan to accomplish these? How will these goes affect them and the people in their life?
Pet: Do they have a pet? If so, what is it? What's it's name? What does it look like.
Wand: What is their wand made out of? What's its core? How long is it? What type of description does it have (supply, springy, flexible, etc.)?
What would she say about herself: If you asked your character to describe themselves, what would they say? Think about how they would say it too.
Back Story: This is where you put the back story or history of your character before the start of the story. Include things like: things they will never forget, things they want to forget, what their childhood was like growing up, what memories can they easily recall, what are their accomplishments and/or regrets, how did they relation with others, how has their life changed, etc.
A bit about the plot: You don't have to give away every detail of the plot but please give enough for the reviewer to understand everything and to see where the character fits in to it all. A basic summary with usually do just fine, but if you want something more in depth then more is needed.
How do you (the author) plan to use them: What changes do you want the character to experience through the story? Do you want them to learn a lesson? Do you not what them to change at all? What issues to you want the character to go through? Basically, how do you want the character and the plot to interact with each other and what do you want to be the finally outcome of the story.

This is not really a profile but more of a template to help figure out the character in a more specific sense. It asks the "this or that questions" and asks for specific details about a characters personality.

I've found both are really helping me get my character's personalities solid when writing my fiction stories. They would probably do the same for fanfic writing as well.
