Thought I'd include a title list of what has been done and what not, or perhaps you'd like ideas and care to use them.
Note: The belts are just examples if you know of more just add them.
Blood
This belt can't be won by only pinfall or submission. You must first make your opponent wear the crimson mask (or make them bleed), then complete the match stipulation, whether it be onefall, submission, Hell in a Cell, TLC, ect.
Belts: PAW Crimson King Championship, WWE:E/EWA Dementium Championship
Submission
Belt can only be won via submission
Belts: EWA Nexus Championship, EWA King of Pain Championship (retired)
Ladders/Climbing
Titles can only be won via climbing up to get it
Belts:EWA Zero Gravity Championship, EWA Nexus Championship
Pure Wrestling
Can only be won in pure wrestling rules
Belts HLPW Phoenix Gate Pure Championship
Hardcore - 24/7
Contested under hardcore rules and 24/7 rules
belts: EEW X-Treme Championship, EWA Mayhem Championship (retired), ICW Kamikaze Championship, ACW Annihilation Championship
Name your own rules
Basically the title holder can make his own rules for the title
belts: EWA King of Pain, EEW/EWA FTW Championship
Dueling
A championship contested dueling.. yes Yugioh dueling
belts EWA Millennium Championship
Cages
Can only be won in cages,hell in a cells, etc
belts GWWF Caged Championship
Weight class
Only specify weight classes can gun for these titles if you don't weigh in, you can't compete
Belts WWE Cruiserweight Championship (under 220),
WWF Light Heavyweight Championship (under 215), CZW World Junior Heavyweight Championship (under 220)
Fast paced
Matches that are usually fast going, that don't really involve technical wrestling
belts: Any X division title, HLPW Dragon Gate Adrenaline Championship
Unsanctioned championships
The concept of championships, and their central role in wrestling, allow for the potential for angles. One such angle is an unsanctioned championship title. These are claimed by a wrestler and defended in sanctioned matches, but are not recognized as legitimate titles by the promotion.
belts: Just about any n00b title rofl , WWE:E Whiny b***h Championship
Renaming
A wrestler may also win a sanctioned championship and "rename" it for the duration of his or her reign.

Random
A roulette is spun to determine the match type
belt: EWA Roulette Championship
Badges:
Win X amount of stipulations to obtain 'badges' in which if you collect all you qualify to win said title
belt: EWA Sprocket Championship

edit: also what exactly do you have to do to challenge for the Millennium Title and if its cool with the winner I want in at The Millennium Title if I can

Title is currently retired as Chrono Clepsydra as it's only champion, this is until Majin returns as the first defense was promised to him.
