Pretty basic. List the Pokemon, its item, and its nature, ability, and moves. Also, you may want to add a description of what the Pokemon's role on the team is. Is it a lead? A late game sweeper? This could be where you describe the niche of your Pokemon.
If you want to, you may even add the Pokemon's stats under the moves, to help us know exactly how powerful, fast, and bulky the Pokemon is. If so, I recommend this format:

Defense:
Sp. Attack:
Sp. Defense:
Speed:
If the team is still on paper (or in thought), you could post the EVs that you plan on using.
Additionally, you may wish to add a picture of your Pokemon, to make it look more "professional." Please, do not include large pictures of fanart of the Pokemon. A simple sprite will do.
Title
How are you going to get someone's eye, get them to actually look at your team? An interesting, creative title! That's how! Try to stay away from titles like "OU Rate My Team" or "RMT Please!" because these are very bland. Try for something that rhymes, is alliterate (Deliciously Devious Dark RMT) and remember to capitalize. These will make your RMT stand from the others, and pull a reader in, especially a Dark user. Right Topher?
Example
You thought I was just gonna let you walk out and try for yourself, letting you fail, didn't you? Well no! I'm going to give you an example of an RMT for two of my Pokemon, to kinda help you see how it's done visually (in case my descriptions or entire guide weren't helpful). Here, I will use two Pokemon from my infamous Sunny Day team.

Ninjask (Heat Rock)
Ability: Speed Boost
Nature: Adamant
- Sunny Day
- X-Scissor
- Baton Pass
- Protect
Ninjask is the lead of my team. His goal is to set up the Sunny Day, then either Baton Pass straight into another member of the team, or start Protecting to gain some Speed Boosts (and then Baton Passing). X-Scissor is mainly for if Ninjask gets trapped, or can kill a weakened opponent.
Claydol (Soft Clay)
Ability: Levitate
Nature: Bold
- Sunny Day
- Light Screen
- Reflect
- Explosion
Claydol is the wall on my team. His niche is to set up both the screens, then to Explode or switch out. He carries Sunny Day in case the sun wears out or Ninjask is fainted.
