Macha was Mia, an NPC who was considered a player in the game.

Yeah, it's true. It actually shows up in the anime, but the episode isn't a direct tie-in with the rest of the series. It came on a bonus disc in the last .hack//SIGN dvd.

In the episode, Tsukasa meets Mai for the first time. Tsukasa automaticly recognizes Mai as Macha, and hands her the wheat piece, that Macha always carried around. Mai doesn't know Tsukasa, but her eyes start to water, and she cant' understand why, and thats where the whole Macha being Mai thing comes into play.

That was a long desciption, sorry about that. XD

If you play the game in Japanese, you'd find it's pronounced "Maha", but can be written either "Macha" or "Maha" (in Japanese, there's often more than one spelling). And while Macha is mainly in "SIGN", in the last .Hack game not only does she (yes, she) appear, but the storyline ties in the fact that Macha IS Mia, only Mia doesn't remember Macha, yet has some of Macha's feelings (therefore the tears when given that thing Tsukasa gives her in the anime).
