No, Itachi would probably still be an ANBU, and would probably never meet Sakura at all...IF they did meet, it MIGHT have a POSSIBLE chance of happening...

I disagree greatly.
One- Sakura and Sasuke are on the same team, Itachi and Sasuke are brother meaning they have a better chance of meeting.
Two- Yes, he is ANBU, which does make him busy but it also means he may have to go on patrol-- which means, you guessed it, he could very well bump into Sakura.
Three- ...I just like the number tree. :3

Hmm... everything would definitely be different.
First of all, due to the clan never being killed, Sasuke wouldn't be the avenging, emo kid he is today... thus, affecting the way he acts towards his teammates. So he would most likely charm his way into a relationship a girl. And unless, he has high standards for women, he'd most likely try to go out with Sakura.
Secondly, depending on whether Sakura and Sasuke dating or not, Itachi would probably rather find a girl his own age. He also doesn't seems to find any interest in women anyway, so that lowers the chances of him even being interested in one of Sasuke's fangirls.
But there's definitely a chance that Itachi will find an interest in Sakura, especially if Sakura finds an interest in him first.
The possibilities are swayed if Itachi didn't leave Konoha. For all we know, he might very well end up gay or killed off in some freak accident.
Jeez, that was a long post...

If Itachi had never left - god, things would be way different.
Personally, I think the fact that Sasuke would no longer be an emo (coughcough) would effect his fangirls a lot and they probably would see him as just another boy - he'd just be really smart (and sloppy) and relatively good-looking. (If Kishimoto made them still like him, that is...erm...)
Sakura probably wouldn't end up crushing on him so much, but if they became good friends, she'd probably come over his house and meet Itachi, this cold figure who's a lot taller than her.
She'd be all freaked and he'd probably end up snorting at her. It would be love at first...fright! OH! Burn. xD My retarded puns are better than yours. :3
Though I also think if Itachi had never left and blah blah blah NaruSaku would have an even better chance. :3

Considering Itachi's not a bad looking guy, Sakura could fall for him, too, and they start going out. Which would, in the end, make little Sasuke-chan the angsty little drama queen he is today, without having to kill the clan. 3nodding Ah, if only this were a discussion on why little Sasuke-chan is the angsty little drama queen he is today xd
