Okay, I'm back. Sorry to take so long to reply. Here is the stuff I've learned over the years. My issues are not trans, but they skirt the areas somewhat and I've had to deal with my own gender issues before coming to terms with being the boi dyke that I am.
Anyways, First you need to find a Gender Idenity doctor or Psychotheripist that under stands gender issues.
Next you will have to spend 2 years as your choosen gender, dressing, name change and what have you. This does not mean going all the way with make up and dresses. Just what makes you feel comfortable. You may have to be stern with your ideas of what the opposite gender looks like because some think you have to look and act completely feminine to be trans, but that is a load of bull.
After 2 years you will be acessed and the doctors will decide if you should start hormonal treatments. Now the reason for the 2 years is because this step can not be altered after starting. Meaning that the changes the meds do to your body are perminate. One should not get hormones from anyone other then a doctor because they can have bad effects if you do not get your blood taken every 6 months to make sure everything is alright. I'm not trying to scary you, but you need to know this because many people have destroyed their lives by doing things on their own.
After the homonal treatments you will be assessed again and then you will look into full sexual reasignment. This is the surgical part that costs alot of money, around $10,000 for FtM. This is the hardest step to get too and the reason why everything takes so long. There is no turning back at this point.
Anyways, There is a quick run down of things. You said you are in Canada so there should be a Gender doctor somewhere. Just do some online searches to find out. When searching for one be sure it is someone you can trust. Talk to them first about anything at all and see what they are like. Questions them about anything and everything.
Well, I think that covers it. Sorry if there are typos. I'm trying to type this really face because we are going to curl up in a bit. I'll try to edit it in the morning.
Well I wish you luck and if you need anything then don't be afraid to ask.

You will have to shop around. Some list the fact that they deal with gender issues and others don't. You'll need to call around and ask if they do. Look for a psychotherapist more then anything because they are more qualified. If you have a gay community centre they maybeable to help you find someone.
Create a list of questions and concerns you have for the doctor. Like what their veiws on trans and gender issues as well as gay issues. If their veiws are different then yours try to get them to come up with a logical reason why they feel that way. Remember this person will be in control of your furture so if you do not feel comfortable with them you can walk away. Just remember to have a back up doctor in line before leaving a doctor because their may not be many other choises.
Most of all remember to stick to your guns on what you want. Some will attack some of the stupidest of things. They are trying to see if you are serious about the path you are choosing.
Anyways, I wish you all the luck
