I think I finally understand the show... here's how I think it is:
Ayu was dreaming the whole time, and the story took place after Yuuichi had found out that she was asleep. The events had taken place in order, but they didn't really happen until Yuuichi told Ayu about them while she was asleep. So Ayu's body was actually just her in the dream, dreaming about all the events that happened as Yuuichi told it to her.

But then how would the events have happened in the first place if they had happened after Yuuichi found out she was asleep?

You might also consider that the visual novel is first person and you know... there's the whole "I thought you had died." and "I did." thing. Well, Kyoto animation strayed from the visual novel quite a bit in some ways.
