Dean loves Kira and can't hide his feelings. He's a little torn because she keeps him at arms length. He believes she's acting this way because of what he had told her. He told her how he tried to replace her with girls that reminded him of her. It almost worked when it came to Cassie, but after she dropped him after he told her what he does. He realized that he can't replace what's lost. You can only try and find it. He also told her that he made a deal to save Sam and now he only had a year left to live. She took it a little harder than he was hoping she would. He can't keep away from her and he doesn't want her getting hurt because of them. He tells her that he needs her to help him in a desperate attempt to show her how he can take care of her and give her as much of himself that he possibly can. Knowing that it was going to kill her inside when his time was up.
