1) Name: Your full name
2) Nickname: A shortened version of your name that you do not mind being called
3) Handle: If you lead a dual life, the name of your alter ego… Usually something catchy (think of Neo in the Matrix) which others can identify you by.
4) Human Name: If your name is something that would be considered strange in human society, the name you will use around them.
5) Gender: Self explanatory
6) Sexual Orientation: Also pretty self-explanatory
7) Marital Status: Either single, married, possessed (a relation on both the physical and spiritual level, like demon to host), or living in (unable to marry their partner for whatever reason)
8.) Location: The name of the Magical Hot spot you currently live in. There are three Hot Spots in this County, please choose one.
9) Place of Birth: The name of the Hot spot you hail from. If you were born more then 100 years ago, we understand that hot spots have died since then. Simply name your country of origin.
10) Can you take a human appearance?: If no, please consider your options before agreeing to move into an all human society. The world is not the same as it was, and humans do not know of the Otherworld- seeing something surreal may upset the balance between the worlds. If yes, consider the conditions of your human form. You must be able to retain it during all human contact.
11) Age: As close to the truth as possible- we understand that after the first few centuries, it becomes hard to remember how old you really are.
12) Estimated DOB: If not an exact date, then at least the year.
13) Viewed Age: The age you appear to be when you take your human form.
14) Race: Please view our prepared list of races. If you do not see an adequate description of your race, please let us know and we will update our list.
15) Viewed Race: The nationality you appear to be in human form.
16) Is your race conditional, or is it by birth?: In other words, were you born this way, or did you become this way through an encounter with the Otherworld? If you are not certain, please look at the race descriptions in closer detail.
17) If conditional, is your status communicable?: In other words, are you contagious? Is it safe to let you live around humans?
18.) Hair: The natural color and length of your hair. Outrageous hairstyles may wish to be dyed or cut to keep from drawing attention to them, but it is not required.
19) Eyes: The natural color of your eyes. Please note if there are any abnormalities about your eyes, such as but not limited to: Unusual color, glowing, reflective lenses, magical properties, and unusual pupil shapes. Such things can be hidden with special contact lenses, which the department will supply for you. Please note also if you wear any corrective lenses.
20) Fur/ Feathers/ Scales: If you are able to transform into an animal or half-animal, please note here the color and pattern, as well as markings on your fur/ feathers/ scales. This includes wings of any kind, tails, and ears.
21) Powers: A description of all the abilities that you are capable of. This includes but is not limited to: flight, transformations of any kind, healing, summoning, black magic, illusions, hypnotism, ect. Please be as detailed as possible, for this information will go on your official DCGI record in the public files.
22) Title: Will be supplied to you by a DCGI official at a later time, so just leave this blank.

25) Wingspan: If you have wings, the span of them. Also, are they capable of flight? And can you hide them effectively in human society?
